The course gives an overview of “Root Cause Analysis”.  It gives an introduction to:

  • The main human factors leading to non-compliances (Dirty Dozens)
  • Explanation of “Root Cause Analysis”
  • Methods for “Root Cause Analysis”, e.g. 5-WHY method
  • Determine “Root Causes”
Objectives of the course:

This one day course is designed to provide the attendees with an understanding of “Root Cause Analysis” based on practical examples.

Upon completion of the training, the trainees shall have an understanding:
– What does “Root Cause” mean?
– What are methods to identify “Root Causes”?
– The 5-WHY method to identify “Root Causes”
